Ideally, everyone who was neither born in Basque Country nor made their debut for a Basque team.

You could set a league to have no foreigners allowed, use team nation, set the Bilbao team's region to the Basque state as defined in the world file (or to all the cities you want to include), and set every other team's region to be the entire world.
It won't catch the "or trained at a club there", but it would put that team to only include players from one region, and every other team would be generic.

Is there a way to do this without creating a whole "country" of the Basque Country?

You can go to Explore World -> Regions Page, hit "Add Region", then add the states Vizcaya, Alava, and Guipuzcoa. Name the region "Basque" and hit enter. Then set the team's Region under team settings to (Other) -> Basque.

I thought this unlikely to be possible as well and I'm glad to know I was wrong too. Maybe there is hope for my "Automatically import real historical rookies" option question after all!
