Trade logic bug?

In OOTP 6.12, with trade frequency setting on normal and favor prospects, I have seen now in back-to-back years (and third time overall in 5 years) an AI to AI trade where one team gets a major league-ready 20-22 year-old 5 star prospect with 0-1.5 years major league time in exchange for a 28 to 31 year old 1-star bench player (or third string player) and one or two one-star single A never-will-be's. In no instance did the club trading the 5-star prospect have a suitable replacement or incumbent.

Since most trades at these settings have some sensibility (both teams have some arguable benefit), my suspicion is that there is a hiccup in the logic that allows this occasional snafu.
