Can't Pitch w/ DTD Injury
One of the starters on my team has a DTD injury (70%, herniated back disk, 6-8 weeks). I left him in the rotation to play (even at 70% he's got mega-mega-VORP), but for some reason when his spot came up (for about half a month) he didn't pitch, but, rather, a reliver started the game for him. I went in and managed a game on my own, putting him in the starting lineup manually (well, he was already there), then simmed the whole thing and he pitched.
I had previously had it on highest rested, and he was skipped over for that reason. I tried strict order, which led to the reliever deal.
The problem continued to happen after I had done the one game where I manually put him in the lineup, when I tried to sim from the schedule screen.
I have a catcher who has a DTD injury as well (80%, inflamed back muscle, 1-2 week) who does play when he should.
