I simmed a new season of 4 games.
My top 2 sp pitched in both games. now the 5th game is my sp 3 I thought this was strange not haveing my 3rd and 4th started pitching in games 3 and 4 of the new season. This is with the lateset patch |

Were there lots of off days? Are you playing as the Padres or Dodgers? Are the rotation setting on the pitching tab set to “pitch highest rested” or “strict rotation”?

This happend at the start of a brand new league with off days.

Same issue here. This was happening in OOTP 24 as well. After looking through the forums, one of the suggestions was to go into League Settings --> Functions and then select the "set all teams to not allow AI Roster Changes". This seemed to have corrected the issue. But it happened again once playoffs began.
In OOTP 25 I have set a 5-man pitching rotation and selected Strict order. I ran a sample of games using the Simulation Module multiple times and for every sim, the game has skipped the 5th starter. I verified that the "not allow AI Roster Changes" is set correctly. I have also verified that the "GM-Only users are always in control of lineups and hiring (legacy mode) under the Global Settings options. I have looked through the forums again and I found posts that indicated this was an issue in Perfect Team back in 2018. It looks like a patch fixed it, but that was specific for Perfect Team. I don't know what else I can do.

Any updates on this? I'm having the opposite problem in a 1920 historical save. The rotation mode is set to "always start highest rested" but it's churning through their 5-man rotations as if it were on strict order.
I'm pretty sure there weren't 5-man rotations in 1920 and not sure why the "highest rested" setting isn't working. In 1920, starters are back to 100% fatigue after 2 days of rest, so the need for a 4th starter should only be for injuries or doubleheaders.
