I've reported this issue before,
most recently in 2018. The issue is still present. Here's what I wrote in my last report:

I reported this issue two years ago, but it appears it hasn't been fixed. When you have the Facegen setting for creating player pictures set to "Upon Player Creation" (in the Players & Facegen tab), it only works as intended for the inaugural draft. After that time, it only creates player pictures on demand (in other words, it's indistinguishable from the On Demand option in the drop-down box for that setting).
This is an issue because it often results in coaches having different Facegens from their playing days. When Facegens are created on demand, many (most?) players never get their own Facegen. Unless a user opens a player's profile or the player has a picture created for some other reason (e.g. appears on an in-game screen, or wins an award that results in the creation of a baseball card), that player will have different Facegens in his player and coach profiles.
