Lineup problem

I am manager and GM of my team and have control of all lineups. Just recently, I noticed that my lineup for a game had been changed to include players who were not in my designated lineup. One player was rested even though he was listed as 100% and was not to be substituted unless tired. Another bench player was included, even though he was not even the designated sub at that position. I manually changed the lineup to what I wanted, but the very next game showed the same subs in the same positions. My bench coach is not listed as having any say in the matter, so what's the answer. Is there a box, somewhere, I'm not ticking and is there a way to eliminate this problem?
