A rarity but needs to be fixed

My fictional league has four divisions, with six teams in each division. In my current season, three teams tied for first place, but only two of them were involved in the playoff for the championship. The third team was eliminated out of hand, even though it had an identical record with the other two. Obviously, the game is not set up to handle a situation of this sort, and though it's a rarity, it definitely detracts from the entire season. Any way this can be fixed?

Game can definitely handle 3 team tiebreaker scenario but I'm wondering if your divison setup has something to do with it. So you had 3 teams all in the same division tied?

I had a rare issue in v20 but because of its rarity most likely wasn't fixed for v21

Three-way tie for Wild-Card

I realize now that what the game did was eliminate the team that began furthest down the alphabet -- Brooklyn and Cincinnati made the playoff, Pittsburgh didn't. This is something I've noticed before when two teams have identical records in the final standings.

Not sure if you've advanced past a point of no return, so this may or may not help, but a couple of things:

1) OOTP has a somewhat new (not sure when it was first introduced) feature that allows you to Reset League to Regular Season Mode. I haven't really messed with this, but it's described as "... in case you want to manually schedule tie-breaking games using the schedule editor." Your scenario would seem to be exactly why this feature exists.

2) In my games, if I have the potential for any ties on the final day of the regular season, I always complete those games without advancing to the next day. This keeps OOTP from scheduling tiebreaker games (if needed) or postseason games. Normally, this is just so I can manually schedule a best-of-three instead of a one-gamer, but also comes handy in those rare instances of a three-or-more team tie, and I want to specify the tiebreaking procedure, rather than go with what OOTP does (or if I need to schedule one or more makeup games).

Thanks, but I've already finished the playoff.
