Has anyone ever seen a relay man throw someone out?

I can't recall ever seeing it happen.

Yes but it seems rare. Happens at third sometimes when the cutoff man relays to third instead of home.

It's rare in OOTP because it's rare in baseball. Think about all of the circumstances that have to line up just right in order for a runner to be thrown out by a relay man. First, the outfielder either fields the ball so deep that he can't get it to the base on a direct throw or else his arm is so weak that he has to throw it to the cutoff man. In that situation, a normal runner is going to be safe, which means a runner who gets thrown out by a relay man will likely be slower than normal. And the play has to be either at third or home - I can't recall ever seeing someone thrown out at second by a relay throw, which means that the third-base coach thought (wrongly) it was a risk worth taking or the runner ran through a stop sign. So you probably need to match up a weak-armed outfielder with a slow-but-reckless runner going to third or home in order for the relay man to throw out the runner.

It happens, but it shouldn't be surprising that it doesn't happen often.
