Unable to initialize facegen

I'm a little embarrassed to have to ask this but how do you get this to work?

I've enabled facegen and 3D features repeatedly but after exiting OOTP as asked and loading the file they both come back disabled.

Any help would be appreciated.

This is what I found in the manual. The SS talks about the app.cfg file.
