Earned run charged to wrong pitcher
A pitcher gave up a two-run home run, but was injured on the same play and taken out of the game. His replacement was charged with one of the two earned runs, despite the fact that the replacement retired the only batter he faced. The hit was charged to the correct pitcher, but the earned run was not. See attached screenshots.

This is because of the "kludge" the game uses when a player is injured. The substitution technically happens before the end of the play.
