Auto Pinch Hitter

Just started playing 16 -- thought I had everything on manual, but I had an injury to a fielder making the 3rd out of an inning, he happened to be up first the next inning and the computer put up a pinch hitter for me instead of prompting me to select one.

What setting am I missing???

You're not missing any settings. This is a known bug that I brought up to Markus during Beta. At the time he said it was something he would have to look at later because it was hard to squash and it was a rare occurrence. It only happens with injury on 3rd out and the injured batter leading off next inning.
