Database Optimizations - OOTP dies with large databases of players. It'll try to sim, but crash immediately, even on adequately powerful PC's. The ability to have 75K+ players would be amazing as it feeds into my second point.
Full Feeder Leagues for North America - Being able to add at least all 298 NCAA D1 baseball teams and a HS league for every state would be great. Adding D2, D3, Canadian colleges and HS's as well as College summer leagues would be even better. This requires a huge database, which is doable, but point 1 would need to be completed first.
Daisy Chain Feeder Leagues - The ability to daisy chain HS to college feeder leagues would be great, as it creates even more realism to the system, which is what I crave.
Shopping Player Packages - While shopping one player is okay, shopping a package for when you want to dump a veteran or contract would be so much easier. Also being able to be offered packages when shopping a player as well.
Umpire personalities - Different umpires have different tendencies. Adding tendencies would add a challenge.
GM to GM communication - The ability for player GM's to communicate to other player GM's in game for online leagues would be useful for those who do not always respond to online messages.
Set Important Dates all on same Weekday - The ability to set all important events (e.g. Draft, IFA's, roster expansions) to the same weekday as to allow for online league sims.
Controlling when FA's sign - For an online league with week sims, FA's often respond to offers and then sign a competing contract within a sim before a team has a chance to counter. Preventing FA's from signing contracts until the X day after the contract was offered would prevent that from happening. An example being if team X offered the FA a contract on day one, they couldn't sign the contract until day 8 if it's a 7 day sim, giving team X a chance to counter any other offers.

Just have college conference leagues like Pac-12,SEC,ACC and put them all in an association.
