How to Add International Leagues to a Fictional Game Setup

Like the title says. I'm starting a new game for OOTP 15, and want to know if I set up a fictional league with all the actual MLB teams and rules, is it possible to add international leagues (ie. Nippon, etc.)? Because right now, I cannot seem to find the function to do so.
Also, as an extra point, how do I change my manager's FaceGen? Thanks!

#2 - Go to your Manager Settings Page - left side, below "Nationality" click on "Picture Functions".
