Cautionary Tale, Part 1
I figured out what I did and I was able to reproduce my error using my "satellite installation" at work. I wanted to use Gambo's updated historical ballcaps and jerseys and I got the bright idea to just copy his folders over my ballcaps and jerseys folders, renaming them as needed. Problem: His folders do not contain the DEFAULT ballcap and jersey files. Nor should they; it's not his responsibility or fault. Which would have been fine except I had another bright idea to refresh everybody's ballcaps and jerseys by going to Player & Picture Options and, instead of choosing (Force) Update / Generate Player Pictures, I clicked Reset All Cap Textures and Reset All Jersey Textures. Duh. Don't ask. Well apparently when you choose to reset uniform textures, you must have those default ballcap and jersey files in place or you get some funky results, as shown below. And apparently this affects some central file like baseball_cap_base.png (see the attached readme file for details - by the way, these notes have to be updated as to file locations but the idea seems to be the same) and whatever cap or jersey you try to upload from that point on is configured in the same garbled manner, for all your leagues. MORAL: Be sure to keep the default ballcap and jersey files in place.

Now, imagine that I had done all of this and did not think to reinstall the game, which is what I ultimately had to do. Imagine instead that I just posted it here or sent it to Tech Support with a title like "Game is scrambling my player pics. Help!" I would guess that it would take quite a bit of digging by forum members or support people to find out that I had deleted the default ballcap and jersey files and then reset uniform textures. MORAL: Be cautious and realistic when you tinker with this game. It's complicated. If you goof around like I did, you need to know what you are doing and take responsibility for your errors. It would be a good idea to document your steps as you make changes, as that may give forum members or support people the clues that enable them to help you. And don't expect miracles. The ultimate solution may be to scrap your league, uninstall the game, and start over.

Good story, good points and good moral. I did something similar a few versions back, but I had backups of everything and could just restore it all
