More variable ejections
It would be cool if players could get ejected for stuff other than just arguing balls and strikes, and charging the mound / inciting a brawl. Arguing other calls (eg close calls at a base, balks, etc) would be the most obvious. It would be cool if other things could incite brawls other than HBP, and if players other than the two instigators could get ejected as a result.

To expand on this, how about possible injuries resulting from a brawl? Even just day-to-day ones like someone wrenching their elbow in the pileup. I've always done stuff like this myself in Commish mode, because otherwise, it feels like the brawls are a little pointless. Nothing happens besides the pitcher and batter getting thrown out. There should be a few extra variables thrown in there, for realism.

Collisions at bases have caused brawls before (the Yankees - Rays spring training one was (directly) caused by someone getting spiked at second IIRC).

Cardinals/Reds - Brandon Phillips talking bad about the Cardinals before the game then tries to be Yadi's best friend at the plate.

Point taken but I don't think these one-offs get to 5% of fights in the big picture.
