Player Expectation

Can anyone clarify the following batter expectations:
Top of the Lineup
Middle of the lineup

Top of the lineup could mean leadoff, 1st or 2nd, or even 3rd.
Middle of the lineup could mean 3-4-5 only, or could include batting 2nd or 6th?
