pitching rotation messed up

I'm in an online league with SP of high endurance.
I've set it up as a strict 4-man rotation as follows:

McDonnell
O'Day
Hines
Shelley

During the last 2-week sim the following pitchers started games. Hines was supposed to pitch in the first game and nobody was injured:

Gaines (mop-up reliever)
Shelly
O'Day
O'Day (2 days in a row!)
McDonnel
Robertson ! (middle reliever)
Gaines !
Shelly
O'Day
O'Day (2 days in a row!)
McDonnel - 4IP and injured, relieved by Gaines
Robertson

This has been going on since the last patch with Hines being skipped Gaines and Robertson getting starts in his place and O'Day pitching 2 days in a row.

Do you have 7-day lineups enabled at all? A handful of times I have seen people get strange starting pitching assignments it is because 7-day lineups were enabled but they didn't set the pitching up there. There was an issue with the starting pitcher position not getting cleared after games were played sometimes which could then leave you with pitchers assigned where you didn't expect.
