Designated Hitter problem-game glitch or rule

Playing the patched version of OOTP 9. While getting a feel for the game, we have been simming through the first 6 innings of games and then managing the last three. 4 or 5 times now into 150 games of 162 game season, we have had a problem with the designated hitter. During the simming either the DH gets hurt or just was replaced by the computer. The game automatically goes to the lineup page. All 9 spots and players are there but the position box where the DH was is blank. A player's name is there but the game will not let us use him or insert another player or pitcher. We can maneuver in that screen like look at the other teams roster or our own individual player profiles, but we cannot get out of that screen, not even able to quit in the normal manner. We have to ctrl-alt-del to quit and then restart the game. Is this a game glitch or a MLB rule or both? Any ideas? Thanks. (Yes we decided to try OOTP again for those of you who might recall the name.)

I think that's the "DH bug" that's fixed in Patch 2.
