Could this be a TCR issue? I don’t even pretend to understand talent change randomness settings, even with lots of insight from the online crew, so I leave it at default. I gather that a higher setting could yield too many surprises like this, or at least a lower setting would make it less likely, more like Strat or APBA purports to be.

I personally don’t play hoping to mimic actual results, in part because I am usually trying to improve a bad team; but also because I like to try out “what if” changes like giving the poor teams more money (or the rich teams a hard cap), just to see what happens. But I gotta have sympathy for anybody counting on a stud reliever who just loses it.

It's not a TCR thing, the underlying talent of Diaz hasn't fundamentally changed, it is just the inherent volatility of relief pitching, especially relievers with dominant stuff and less than elite command. Essentially this exact thing has already happened in real life with Diaz a couple years ago and it has happened with Craig Kimbrel, Greg Holland, Blake Treinen, Adam Ottavino, Dellin Betances, and dozens more. Sometimes they bounce back, sometimes they don't.

People really need to understand sample size, not just in OOTP but in real life baseball. Baseball is probably the most random sport on a game by game basis, hence why we need so many games in a season. Also why playoffs seemingly make no sense so much of the time. If you put the worst team in the league into the playoffs, they'd still have a feasible chance of winning the World Series just through luck and good timing, which is probably not true of most other sports.
