Playing with the Marlins against the Mets, Beckett just hit Hidalgo with a pitch immediately after a HR...the benches emptied(first time I've seen this...that was pretty cool), and it then said that the ump ejected both the batter and the pitcher.
I was forced to replace Beckett, which I did, but the CPU did not replace Hidalgo, and he continued to play in the game as tho nothing happened. Also, they still had players left on the bench, as it was only the 5th inning, and they later Pinch Hit for their pitcher with Karim Garcia. Score: Florida 5, New York (N) 2 As you can see after Hidalgo was hit it shows the new pitcher, Bump, but Hidalgo was allowed to remain in the game. Has anyone else noticed this happening? I've never had a fight occur before in a game I was playing, so I don't know if this was a random occurence or if it heppens all the time.... Ack...pitcher was Burnett and not Beckett...I am always getting them confused...

Thats a known issue, or was in Version 5.

I just had a similar problem, except that my player was the hitter. When I made the change for him, I inserted another player into the lineup to play his position. However, the runner now on first is listed as the pitcher that the computer had replaced. So, now the same player is on first and pitching for the other team. Here's a screenshot of the problem.

Not quite the same, as it's the opposing team's pitcher that is placed on first base and then in fact later comes around to score. Kind of funny to see that Brian Meadows scored while Brian Meadows was pitching. Anyway, it's probably in the same general area as all of these other ejection bug issues, just wasn't sure if it was worked on lately and whether this was a new one that popped up after their efforts to try and fix it.
