OOTP 6 Possible Injury Bug

On a 2-2 pitch with 2 outs, Scott Williamson struck out Rafael Palmiero and was injured. The PBP said that Williamson left the field with the trainer.

I was never sent to the lineup screen to put in a new pitcher. The inning just ended and my team went to bat. At the bottom of the next inning, Williamson was still on the mound. He had the normal color font and seemed ready to pitch. I removed him because I didn't want him to be injure himself further.

I'm not sure if this is a bug or just a new way of handling injury replacements.

Hasn't this been a bug for the last few versions?
