Which dificulty level and why?
I want the hardest challenge but without the game making anything unrealistic. In other words, I don't want a 7 run comeback in the ninth inning against me simply because I have the game set at ALL-STAR or whatever the hardest level is.

I set it to the hardest level, and never initiate trades (will only accept trades offered by the AI).

Most people find the hardest trade levels to be reasonable (heavily favor prospects and very hard) and even then, sometimes they have additional house rules to prevent rip-off trades.
I don't think the difficulty affects gameplay, except in the sense that an unrealistically great team would be more likely to come from 9 runs down.
