Ohtani injured during game - how to replace as pitcher and hitter?

Ohtani got injured as my starting pitcher - I can't figure out how to proceed at the starter and DH position. Any suggestions?

what happened:

I tried to fill in a pitcher in the pitcher spot and a new hitter in the DH spot but now the game won't let me proceed. The DH spot doesn't have an active option for the POS setting. Do i need to replace that spot in the lineup with a pitcher? If so...how does that carry over the Current Pitcher setting?

But it's the same line-up spot so that new pitcher will appear as a "P" in the line-up. Once the "P" is selected, that should be all nine players needed.
The DH position is forfeit, I believe, in the scenario as you describe it. That new pitcher is a batter - though as if it were an old NL game you can PH for him when his turn comes up.

Why would Ohtani leaving to an injury change whether the team is allowed to bring in a new DH+P, vs when he leaves the game normally and is allowed to keep DHing?

Even when he doesn't keep playing DH, the desired action by the OP should have been doable as it happened in Ohtani's final pitching game this season:

I'd reckon that there is an order of events that has to be followed for OOTP to recognize what is being done is allowed. It's never happened to me in game, so I've not tested the various possibilities. It's not hard to see a situation though where doing X first as a substitution forfeited the DH spot according to the engine whereas if one had addressed Ohtani's other position first, it would have allowed one to make multiple substitutions preserving the DH and also getting a new, different person to pitch.

Heck, there may even be an order of events when a manager irl goes out to the umpire to make the substitution that he has to explicitly explain what (to 99% of the populace) is a logical and obvious splitting of the positions.

Did you fill in the new DH in the lineup first, and then when needed select a new pitcher?
