Injury Recovery for SP
Anybody have an idea how long pitcher rehab needs to happen after a long term injury?
I have Kyle Bradish from Baltimore in 2025 who missed the entire season as he got injured during Spring Training. It's early September and he just made 2 rehab starts. I called him up after that because we were just 2 games from a playoff spot, and he's gone 0-3 since, with a 9.75 ERA and 18 walks in 16 innings with only 2 K's. That is unusually bad! I re-scouted him after coming back, and he didn't lose any skill, so maybe I didn't give him enough time making starts in rehab? I've obviously had to pull him from the rotation. |

I forego rehab assignments. Usually, the player is OK after a few games, for SP, use him out of the bullpen till the rust is off.

No firm information but on rehab I try to get a SP at least 3 starts and maybe 12+ IP on rehab. As to skipping rehab it is my understanding, and I could be wrong, that a player on rehab is less subject to reinjury than if he is activated without rehab and has accumulated rust. He may also struggle performance wise until he knocks off the rust so it is up to you if you'd rather have those 'bad' innings in the majors or minors.

How did he pitch in the rehab starts? I would have done as you did; but he wouldn’t come up unless he pitched effectively at AAA in rehab.
Also, there is the issue of stamina, and working up to a decent number of pitches. It is unlikely that a SP could be ready for five innings or 100 pitches after only two rehab starts. Remember, he is basically back at the beginning of Spring Training. With this timing, many MLB teams would only use the SP as a reliever. He just won’t be ready to pitch deep into games. Bradish has a live arm, and could be a valuable piece in the bullpen. I would wait until next season to put him back in the rotation.
