Playoff roster eligibility

Hi, another dumb question here.
I've been playing the games through a bunch of seasons and I've been fortunate enough to make the playoffs a few times. However one challenge I have found is that sometimes I end up short in my playoff roster because I find a player I want to put on the playoff roster is determined to be ineligible.
I had thought as long as you were on the 40 man roster as of Sept. 1 you were eligible for a playoff roster but I keep finding this ineligibility issue. What are the rules for eligibility? Is the roster set by who is on the active 25 on Sept. 1? That doesn't allow for sudden injuries or the potential need to replace a struggling player with a hotter one.
Any advice on roster management would be much appreciated.

I don't enable the playoff roster part of the game so don't have any experience. Does OOTP not follow these rules?

According to the manual the rules OOTP22 uses are displayed on the playoff roster page (which I can't access since I don't activate that feature). Anyway I hope this help a bit and maybe someone who knows more will come along and post.
