Incorrect Seeding

As you can see from these pictures, the seeding for the playoffs switched the 3 and 4 teams based on regular-season standings

I've tried it with a couple of difference setups - 16-team, 2-conference custom league like this appears to be and the default 18-team, 2-conference, 4-divsion league, both seem to work correctly. Anything unusual in the way you have the league structure set up? Was the game started with the most recent version of FHM8?

I don't think there's anything weird with it. It wasn't started after the most recent update.

Switch the playoff format to "8 Teams, 4 per Conf", it's currently on "8 Teams, 4 per Conf (DEL 2019)", which does the 1v3, 2v4 matchups.

That rings a bell. I think I tried that because I had the 8 teams, 4 per conference the first season but it ignored the conferences come playoff time.

I'm hoping I just screwed it up last season and chose the wrong format but this seemed weird

I have confirmed in simming another season that the "8 teams, 4 per conference" playoff format isn't working in my save. Once again, it took the top 8 teams instead of four from each conference.

Can I get some help on this, please? I've tried a bunch of different fixes in a quickstart and none of them work and some have revealed more issues.

Try switching the format to Custom, check the "Conferences Separate" box, and choose "Quarterfinals (8 Teams)" as the Start Round. With that league setup, that will give you the top 4 in each conference and a 1v4, 2v3 starting round.
