Fleeting position changes

I've noticed something in my box scores that shouldn't be happening. When I'm adjusting my lineup after making player substitutions, the game is picking up when I accidentally place someone in the wrong position while setting my defense. For instance, if I pinch-hit for two players, my catcher and my left fielder, and accidentally place the new left fielder at catcher before correcting myself, the box score will show him as having been inserted at catcher, then moved to left field. This should not happen. I would suggest that the game should not apply any position changes until I select "Return To Game".

On that note, is there any way to cancel out of lineup changes that I've made on the substitution screen? As it stands, it appears that if I bring in a player, then change my mind and decide to insert a different player before leaving the screen, the game thinks I used that player anyway and that player is no longer available for the remainder of the game even though he never actually entered the game. What am I missing? (Note that this would likely also be solved by addressing my first issue, assuming that I'm NOT missing something.)
