Lineups not working

We're in Spring Training of our 15th season in our online league which I am the commissioner of. I usually set my lineups so that Utility 1 starts every 3rd game and Utility 2 starts every fourth game. This has worked in seasons past as far as I can tell. However, this season Its only starting Depth Starter and Utility 1. Utility 2 gets no starts and even my top pinch hitters are not being used.

I have 7-day lineups set to no and pinch hitting strategy 3/4 of the way to frequently. I reset all player strategies to default prior to Spring Training.

We have no coaches and all of my manager settings are set to me. What's going on?
