Creating a USA vs The World type tournament?
Is this possible? I want like a Presidents Cup style tournament to be set-up every few years

Basically I'd like a mix of regions/nations in my tournament

Well you could do it by having region vs region where one region has a single country, USA and the other region contains all the other countries you want. Im not sure if there is a way to add regions in game but its easy enough to do via updating the world_defaults.xml

You might be able to do this using US vs the DSL region setting (i think it adjusted last year to include all nations but US)

I'll tell you right now the US will slaughter most countries except for japan and the DR. You'll need to play with the modifiers a bit to make it more competitive. While a good piece of the world is on par with the US when it comes to amateur and beginning pro level competition once you get into the major it's just a numbers game. The US has more talent (not better) just more in terms of numbers.

this is for a fictional league, US will still slaughter everyone I'm sure but I've got a 6-team Cup setup now. Latin America, Canada + Alaska (hoping it's just Canadians), USA, Europe, Asia, and Southern Hemisphere. will see how it shakes out after the season when teams select players...I manage Southern Hemisphere and can already see the dual nationality guys are generally not playing for me, even terrible Americans who will have no chance of making the USA team. anyone know how this works?

Nope it does not reload the file after you change, you will have to reload it but that shouldnt cause issues just adding a new region. To reload it I think you go to settings > database > actions > load database from world defaults.xml. Dont have the game in front of me now but it should be easy enough to find. Just make sure to backup the game before doing anything just in case.

Would you be able to do this with, say, high school kids? I would love to do a summer showcase of the best high school players from each state competing in a giant tournament. Like a World Cup but each team is comprised of kids from the state.
