Wrong Team With Home Ice Advantage
I really thought this was fixed. I'm playing my fictional league playoffs and the Western Conference's 8th seed Seattle Seawolves beat the 1 seeded San Jose and then beat the 2nd seeded Minnesota. They are now in the conference finals against the 4th seeded Edmonton team. They have been mistakenly given the home ice advantage in the conference finals. See pic.

This has also happened in my junior league. The 8th seed defeated the 1 seed, then beat the 2 seed and was "rewarded" with home-ice in the conference finals.

I will send you a pic of the standings later (I'm at work now). It appears to me that the playoffs are "bracketized " if that's a word. The 1, 2 7, and 8 seeds seem to be in one bracket and the 3,4,5, and 6 seeds are in another. The winner of the 1,2,7,8 is awarded home ice in the conf. Finals. Most of the time this will work out because normally the 1 or 2 seed will win 2 playoff rounds. I suspect this "bracketization " only occurs in custom games.
