Most Realistic Universe
What is the best settings for a realistic universe with all leagues active? Should I disable international FAs, etc. in the MLB and just allow players to come from the individual leagues naturally?

To answer the specific question, yes, I would definitely disable the established International free agents. Those are there to simulate the other world leagues when you don't actually have "real" versions of them running. I'm honestly not sure what other world leagues there are, but you may want to leave the amateur free agents/signings if there isn't a real world mechanism to get players from the Dominican Republic, etc.
