Injury on the last out of an inning
If a player is injured on the last out of an inning, I am forced to change him as the games tells me the lineup is "not complete" if I don't.
However, IRL I don't have to place a sub for him until he either bats or takes the field. For example, if my pitcher is batting fourth the next half inning and he is injured on the last out, I am forced to sub him the moment the last out is made. However, I should have to option of using a PH if gets up to bat or placing a new pitcher if I have a 3 up 3 down inning. |

Good point on the rule violation.
One of my pitchers was credited with 0 IP as well even though he didn't face any batters or record an out, which is clearly against rules. |

Is there no way to fix this?
This is very frustrating because again, I had to sub for a pitcher after the third out only to have a PH hit for him when it was the pitcher's turn up to bat. This will never happen IRL. |

Just had this happen to me again yesterday towards the end of a long game. I had to waste a position player in the pitchers spot to continue. Blah...

Pitchers must face one batter or retire the side. Because he entered during the half of the inning in which his team was defending, he retired the side.

Yes, you are right. A substitute pitcher must face one batter or retire the side.
But if the third out has been recorded by the previous pitcher, shouldn't the side already been retired? Anyways, I don't think OOTP can strictly enforce that rule anyways. While I can't sub a P for a substitute P, I can swich him to 3B before the at bat finishes or the side has been retired and have my 3B pitch the rest of the AB.
