The OOTP AI made some pretty bad decisions in his simulations.

When I played the 2016 season out as only manager, Phillies GM Matt Klentak was fired after the season was over. I purposely played as only manager to see how the new GM would do. The AI didn't have him do very well, I guess.

Two of his first bad moves were sending Freddy Galvis to the AAA team despite hitting over .300 at the time. And keeping him there.
Next bad move was releasing 2B Caesar Hernandez. We got nothing for him. Just let him go. And that was just the start.
