Importing a game that uses an edited world_default XML file

When importing a game that utilizes regions I've set up in an edited world_default.xml file, does it matter if the XML file is edited before or after the import? I have a game where I've added my own US regions that aren't in the default file.

Well, when they are already in the game you don't have to do anything for that particular one. If you want to use it in other games, you need to replace the default one that comes with OOTP 17.
