Logo glitch?

I have loaded all major league teams with new logos and uni's from the mod section (Nickname project v1.1-1.3) but the game continually reverts to the old generic style. It is random, one or two teams at a time, but very annoying to have to go back and change logos, caps, jerseys each time I spot the error. Some teams I've changed three or four times. What is causing this and how do I fix it? I've tried saving directly after fixing but it doesn't seem to work.

Just throwing ideas out...

Do you have logos in the OOTP logos folder and are they named properly?
Is it ML teams or minor league teams?

I have seen most but not all of my ML minor league teams revert because I deleted all logos and uniforms from the OOTP logos, jerseys and caps folders. No MLB teams have had this problem nor have teams in my ***, CBF or EBL.
