Messing with Name/First Name Files

With OOTP11 can you just add any name that isn't already in the file just by following the "coded" pattern of the file? What if you were to duplicated a name, like if you added one of the name mods from the last game. From what I can see you should be able to add whatever new names you'd like to add and they'll come up at some point in the game. Is that correct?

You are correct. One of OOTP's strongest points is it's customability. You can modify your names files as much as you desire and in any way that suits your fancy(see the manual). Don't forget to re-import your name files after changing them (on the Game Setup screen). Also, a good idea to backup your original name files before making any changes.
