You must make sure that you save this file as a .PNG.
Go into your team options where you can change the team info (city name, team name, etc) and click on 'edit team logo and uniform' (or something to that nature, I don't have the game in front of me) then click on load a uniform template (same with the cap template) and browse to where the .PNG file is saved.

Once you do this, don't forget to click 'Update all players on team' so that everyone is wearing the new uniform. You all my instructions just confuse you, maybe someone who is looking at the game can help..lol

Johannesburg ZULU. My South African fictional team . Be sure to keep a look out for our upcoming dynasty thread!

Haha, yeah it came out alright considering it was my first one. I actually really enjoyed making it.

It is a million times better than the first uniform I made.

That means a lot. I've really enjoyed looking at your jerseys in the Mods board. Now to my affiliates! Thanks again for all the help and kind words.

Great job, but I'm confused about one aspect. I can't reconcile the initial post-up with the split colors in the jersey itself. Is that a specific setting?

No I ended up changing the jerseys. In my initial post, the "JS" on the hat was off-centered but I saved it so that I couldn't alter it. I ended up making a completely separate set.

Here's what the template looks like for my split jerseys:
