bench clearing brawl, but no one ejected?

just had my first bench clearing brawl today after playing over a 100 games. The play by play said that both the pitcher and hitter were thrown out, yet neither was removed from the game.
has anyone else seen this before? I have suspensions turned on and i was managing the game one-pitch against the computer.

Are you running with Patch 1?

Did the teams have any players left on the bench/bullpen?

i am running patch 1.

there were players left on both benches/bullpens. here was the situation. the starting pitcher gave up a home run. i brought in a new pitcher via a double switch, and he proceded to give up a home run to the first batter he faced. the 2nd batter he faced, he beaned. this is when the play by play said both the batter and the pitcher were ejected, but neither were actually removed from the game.

This has also happened to me with Patch 1, but it is clamed to be fixed in Patch 2 (OOTP 9.1.0). Hopefully that is the case.
