Is there something up with coaching contract extensions, or is it just me?

I have noticed on and off that I am without a coach at the beginning of the season. Which is ok, because sometimes I simply forget to sign new ones for ones I lose.

But ... for the last 10+ seasons, I am 100% sure I am losing coaches I sign to contract extensions.

I say so with certainty because after a while I just became obsessive (and also realized I couldn't find much better personnel than what I already had) and just went down the list, offering an extension to anyone I could.

This new season in particular, I am without a Manager, Bench Coach and Hitting Coach. And I am fairly certain I signed them all to extensions.

Am I missing something here or is it broken?
