Irrevocable vs Revocable

I know the difference, but what determines which one a player will be when placed on waivers?

Also, how can you tell ahead of time (probably just use the answer to the question above.)

...and i believe the player is out of options once he's sent back down to the minors for the third time.

Technically, it's for spending time in the minors for the third season while simultaneously on the 40-man , but essentially yes.
