If you have it on when you import historical players, the game will make righty batters better vs. lefty pitchers and vice versa (and pitchers better against those of the same handedness). This is usually, but not always, the case in real life. Since the game can't import real splits, it assigns generic ones to the players.

Without the option, I believe everyone gets the same ratings vs. lefties and righties, although I'm not sure.
