DaveHorn's HFTC Export Tracker question

Hey all .. i recently had to redo my league site and lost the old tracker I used so switched to Dave's. If Dave or anyone can help me on this next question I would appreciate it.

here is my sites export tracker http://outlawuniverse.net/obl/teamexports.php

The new and old file is not working. . what do i need to do to get that to work?
