Pitcher remaining in game after being pinch hit for?

After pinch hitting for Jon Lieber with Matt Kata in the top of the 7th inning, Lieber remained in the game for the bottom of the 7th. He recorded one out before I noticed and replaced him.

Has anyone ever seen this happen before? I'm using the latest beta patch for 6.5

Here's the game log below for the 7th inning.
