I don't want to speak for the devs here of course but to me (and this was confirmed publicly by Lukas so I don't think I'm speaking out of turn here) the biggest changes I've seen have come under the hood: there was a multi-year effort to fix historical defense and strategy, for example, to make the game more realistic for that period (to the point that, at least according to some of the data, the pBABIP rating no longer applies to very many people at all, at least not that much). I think that was a thing the guys who work on the engine have been putting a lot of time and resources into doing and it should also have the effect of improving play for modern and fictional games as well.

The above isn't sexy and it's not really a thing you're necessarily going to see through screenshots, but I also feel like it's exactly what so many of these people who are like "OOTP should just fix what they have instead of adding on new features" have been wanting. Last year I think adding "hard mode" to trading cut down on a lot of the worst excesses of the AI by not allowing you to add and remove players from a trade and see what the opposing GM thought of them until you set something in stone. Again, not glamorous but exactly the kind of thing people say they really want.
