So, there is no distinction between single A and High A+. Even when I go into the game and look at the level of players in the leagues, it is essentially like having six single A leagues, rather than like 3 A leagues and 3 A+ leagues like in real life.
Does anyone know how to fix this issue? I would like to make my "fictional" MLB league are real as possible, so I would want the distinction between High A and low A to exist.

Not trying to be a know it all here and if you have already tried this, please forgive me. When you go to the create fictional league menu, on the page you select how many teams are in your league, if you look to the right side of the screen, the option will be there to use the MLB template to create your league. If you choose the MLB option, you will get all the real mlb teams, minor league teams etc, and all the players will be fictional.
