Start with the TV delays and it'll speed the game up considerably. Go to banners/in-game ads and drop all of the commercials. No extended half-inning break. Give the pitcher a short amount of time to get to the mound, 6 pitches and go. Commercial breaks come after the inning is over, during a pitching change or a review.
