The biggest factor is certainly his radiation treatment. I have a grandmother of almost identical age, and she recently underwent six weeks of small dose radiation treatment in an attempt to shrink a small, relatively benign tumor.
It was nothing like the normal radiation treatment that is used to treat cancer, and the process weakened and exhausted her to the point that she sleeps 20 hours per day, hardly eats anything at all, and has declined dramatically.
Paterno's treatment has probably been much more aggressive, so the outcome is probably more severe. It's probably too much to handle for someone who is already frail and has had other health issues.
