Well, I agree we'll probably never know how all this got mangled and then got fixed, so to close the loop here:
Since you are running the game in your own chosen location (which I do as well), you should have OOTP folders/files in only two locations:
1. Everything you can see should be in your chosen game folder location.
2. You need to keep the OOTP folder(s) that you see in your Documents folder, but they should be empty (or appear to be empty). The files you see there now are duplicates and can be trashed.
Obviously, you should have been able to choose your OOTP folder (the folder, not the app) in your game folder as the place for the patch, in which case there would have been no problem. Hopefully, if there is a future patch, this will be what happens for you.
