Couple of things

1) Double switches. PH for pitcher and the PH stays in the game as a fielder in the 9 spot, while the new pitcher goes into the slot of the fielder being replaced. I was unable to make this happen: kept getting a dialog box saying ‘do you want [PH] to be the new pitcher; he will bat in the #9 slot.’

2) If an injured player comes off the injured list at the start of a playoff series his 10 days on the DFA list can run out forcing you to release or trade him. I was unable to find a way around this and ended up settling for a bad trade before losing the semi-final series in seven games. (Probably it was the AI that put him on the injured list in the first place instead of keeping him on the expanded roster, but losing the player seems a bit harsh as a penalty.)
