Failures

Tony Mullane
One of the great pitchers of the early years of baseball was Tony Mullane.
Mullane was 284-220 while playing for 8 different ball clubs. In this world
he did not pitch at all. 1881 saw him become a minor league free agent.

Joe Start
Joe lasted only seven years as in real life he saw action until 1886.

I'm not sure I would call these failures. You have the game set up to use Development and whatnot don't you? If you do, right away things will be different.

How do you know when to expand the league or do you just set it to random?

Using the real players of the time. I also use the auto expansion that is built into the rules.

I let OOTP engine do all the work. So when Babe Ruth debuts in 1914 he wil imported as pitcher. The next year the engine will see what he did in real life and import as a pitcher until the 1918 season and he will imported as a left fielder.
